Cosmetics, supporter packs (they put them up a few months before each expansion, and supporters get exclusive cosmetics, forum titles, and stuf like that), and some very minor in-game stuff - nothing that affects character power, but the main one is several forms of extra stash tabs (some that hold more but only certain types of items, ones you can rename. Ones that are color coded, or combinations thereof.). You dont NEED them if youre good at paring down your inventory (the default is like 8 or 9 tabs IIRC), but, honestly, ive thrown maybe 20$ On them, and since ive got well over 120 hours into the game, and its a genuinely good game, i dont regret that at all. Honestly, if i still played regularly, id probably have bought supporter packs just because theyre a good company that listens to he players and produces a quality game, and in the long run the $/fun times ratio would still be absurdly good.

In fact, the reason im not currently playing is because the choices can sometimes be overwhelming, and i got kind of ... lost? Its a genuinely good game, though, and the studio is very in touch with their player base and they produce TONS of new content all the time, and have a raft of specialist “Leagues” and Playtypes (the Leagues are usually time-limited, and the most popular ones stick around as alternate full-time server types, like Solo Self Found (cant group, cant trade), which the Hardcores really like) that change the game in sometimes significant ways. Theres a LOT to do, but it can get very complicated. Its not simplified down like D3, its like D2 and they went “but what if we add like ten times more variables/stats, plus pseudo-classless, oh, and Skilldrasill” - and that is where they STARTED at the game launch and theyve added even more stuff since. Its not for the faint of heart. Thankfully, the forums are friendly and there are lots of guides out there.

Note that there are issues with it apparently and according to at least one user it requires 10.15, so I don't think I'll be giving this a shot anytime soon given that I'll lose access to all my 32 bit applications if I install Catalina.

...Then I realised that I can't input a @ using my Mac keyboard on Geforce Now, so I can't actually log into the game.. Boo!
